iLA is seeking Clinical Team Leads with experience delivering Aged Care Assessments to support our expansion across Western Australia as a part of the Single Assessment System.
THE ROLE
You must be a Registered Nurse, Occupational Therapist, Physiotherapist, or Social Worker with proven experience in conducting aged care assessments. Key responsibilities of this role include:
- Work with iLA’s Assessment Service Delivery Manager to expand iLA’s existing operations and establish and Aged Care Assessment Service that meets the needs of clients across the whole of Western Australia.
- Lead a team of Assessors working across metropolitan, regional and remote areas of Western Australia, through co-ordinating, overseeing and planning for delivery of Aged Care Assessment Services.
- Ensure high client satisfaction and work practices / systems that are conducive to productivity, safety, continuous improvement, and employee engagement.
- Co-ordinate Assessor resourcing and service delivery in consultation with other relevant stakeholders to meet operational needs and KPIs.
- Supervise the activities of team members, including mentoring and coaching, development and training and allocation of duties.
- Ensure the team practices culturally sensitive assessments for groups such as Aboriginal and Torres Strait Islander individuals, those from CALD backgrounds, the LGBTIQ+ community, or the homeless.
- Promote healthy ageing and well-being using a ‘wellness and reablement approach’.
- Collect, analyse and report on operational performance data as required
- Conduct Aged Care Assessments, Triage and Delegation services.
ABOUT YOU
The ideal candidate will be self-motivated and inspired by a passion to make a difference to the lives of aged and vulnerable members within the community. You will have confidence in your ability to lead and develop others, be flexible and solution focused, be able to manage competing demands, and work effectively as a member of a team to deliver to operational requirements.
You will also possess:
- A tertiary qualification in Nursing, Social Work, Occupational Therapy, Physiotherapy or a related health field and current unrestricted registration with AHPRA or eligible to be part of a relevant professional association
- Experience as an Aged Care Assessor, using a client centred approach
- Experience as a Team Leader within an aged care setting desirable.
- A passion for promoting independence and well-being among older adults
- Excellent interpersonal and communication skills, both written and verbal
- The ability to work flexibly and complete aged care needs assessments, delegation and/or triage duties as required
- A willingness to complete training requirements specified under the My Aged Care Workforce Learning Strategy 2023
